Cloud-hosted PMS platforms shift those responsibilities to the vendor but add internet dependency and recurring subscription costs. SQL Performance

Dentrix and other server-based PMS platforms run on SQL databases that degrade over time without maintenance. Index fragmentation, transaction log growth, and outdated statistics cause queries to slow down — showing up as lag when opening patient charts, running reports, or processing claims. We schedule regular database maintenance during off-hours to keep performance consistent.

Backup Strategy

Practice management databases contain irreplaceable clinical and financial records. Our backup approach includes daily verified backups, encrypted offsite replication, and periodic restore testing to confirm that recovery actually works when you need it. For practices running local servers, we also maintain bare-metal recovery images that can rebuild a failed server without reinstalling the PMS from scratch.

It depends on your situation. Cloud platforms like Curve Hero and Dentrix Ascend eliminate local server management, provide automatic updates, and allow access from any location. The tradeoffs include ongoing subscription costs, complete dependence on internet connectivity, and less control over data hosting. Practices with reliable high-speed internet, multiple locations, or aging server hardware are often good candidates. Solo practices with solid local infrastructure and no plans to expand may find the switch adds cost without proportional benefit. We evaluate both options against your practice's specific needs before recommending a direction.
Dentrix performance issues usually trace to one of four areas: the SQL Server database needs maintenance (index rebuilding, statistics updates, log management), the server hardware is undersized for your patient volume and user count, the network connection between workstations and the server has latency or bandwidth issues, or the workstations themselves lack sufficient RAM or are running too many background processes. We diagnose the actual bottleneck rather than guessing — sometimes it is a database issue, sometimes a $200 network switch is the fix.
Daily at minimum, with the backup verified through an automated integrity check. Transaction log backups throughout the day provide additional recovery points — allowing you to restore to within minutes of a failure rather than losing an entire day of work. Backups must be stored offsite (encrypted cloud storage or a physically separate location) so a fire, flood, or ransomware attack that hits your office doesn't also destroy your backups. We test restores quarterly to confirm the backup chain actually produces a working database.
Data migration between PMS platforms is possible but requires careful planning. Patient demographics, clinical notes, treatment history, and financial records can typically be exported and imported, though the level of detail preserved varies by platform. Imaging data stored in DICOM or standard formats migrates cleanly; proprietary formats may need conversion. The migration process should include validation checks to confirm record counts match, patient-image associations survive, and financial balances reconcile. We manage the full migration process including parallel running periods where both systems operate simultaneously until the new platform is verified.
